Did you also put a G92 E0 after your extrusion code...?
Your E values are absolute, does your extruder wheel runs reverse after your startup code...?

I really like your explanation!
Solution from my point of view:
In the gcode:
Use M92 E28 for 'old' slicers
Use M92 E865'8888 (you should calibrate this value!) for new (volumetric) slicers.

Did you try Cura, there is an automatic firmware update in the startup wizard...

Not the solution, but can work: If you know the jump speed, you can go to motion and set the Vmax X and the Vmax Y. Then it will never go faster than that...
(I use the Vmax E many times, that is for me the most crucial)

It are probably the pulleys of the motors (onthe axis and the shaft).
Be carefull with turning up the power!! A quarter is a lot. Maybe they where up to much already and then you can get the problem as well. Stepper overheats --> stops --> cools down --> starts again.
(can somebody change this away from software/cura, it is a hardware problem...)

Or place the objects diagonal. I printed taller objects that way with the planner...

just my thoughts: If it extrudes 'half' of the filament, maybe the jumper settings on the extruder stepper driver are not set correctly....
Or is the filament skipping/slipping, you can feel that by hand.

was there maybe a gcode m220 in the file...?! that sets the the speed in some marlin versions

we have an ultimaker in the lab as well with the 8mm rod
we had a problem several months ago, the nut was becoming a ring, the thread was totally worn out, maybe this gives problems in the big variations
solution is to replace the nut and grease the rod very well
maybe it helps
