To diagnose this specifically it would really help to have the Cura project file (.3mf, in Cura just go to File > Save Project after you set everything up).

Based on your picture:
The tops of the holes get support because the angle there is actually sharper than you might think and would require filament to print past the maximum distance it can overhang. Small holes you can usually manage without support (so put a support blocker in) but larger ones, the distance is just too great.
The... hand thing? At the centre-ish - the outcroppings are probably too small to be supported automatically (whether you can actually print them really depends on your printer and filament) but to get them supported try reducing Support > Minimum Support Area.
The bit at an angle sticking out that the top is probably within the allowed overhang angle. Try lowering Support > Support Overhang Angle. This will make more of the model get supported overall.
