Try to confirm that the stepper motor drive belts (attached to stepper gears) for the Y and X are close or the same in tension. I had a similar problem and my (y) stepper actually moved from it's original location. It drifted up just a few mm and that was enough to give the belt slack which to the best of my knowledge gives you what you have in the picture.
