Water based acrylics used by plastic model builders works well. The usual Vallejo, Games Workshop, ArmyPainter paints etc. I'd use a primer, yes.. couple of 3-4 coats of filler/primer spray and a bit of sanding also gets rid off layer lines. Check automotive primer spray cans.
If your kid is still young enough to stick everything in the mouth, maybe even consider a food safe resin coating on top.
