The very top or bottom layer of skin, depending on whether you're doing a roof (something visible from above) or floor (something visible from below) is considered the visible layer, because it's the one you're actually going to see on the finished model.
The rest of the top/bottom layers for that particular bit are considered fair game for Not on Outer Surface with the idea being that the layer/s above will smooth out any strings so you don't see them.

Every combing setting (other than Off) avoids all inner and outer walls.
All just makes it avoid walls.
Not on Outer Surface means as well as all walls, it'll avoid the top layer of skin (where it's a roof) or the bottom layer (where it's a floor) - if you have the preview set to line type, skin is yellow.
Not in Skin means it will avoid all walls and all skin, not just the visible layer.
Infill Only is exactly what it sounds like.
