Where's the reply?

Look at the image. The church bell says: You've got a reply. But the topic says: No replies. it's not there. What happens?

There are still fake notifications. Its on the to do list of the developers (according to @SandervG) I have the idea it's getting better though. Next to that there also still a lot of double notifications.

There are a few possibilities, like a deleted reply, an unapproved reply or spam which may trigger a notification.
