Mushrooming between coupler and hotend.

Hey,

So this all started when my stepper started to skip and my printer was not extruding enough filament to fill the layers. so I took the hotend assembly apart and did some tests and found that the filament was mushrooming inside the coupler or somewhere between the coupler and the nozzel. Does anyone have any suggestions on how to fix this or any other ideas of why this is happening?

100% fan

70-100% flow (I also tried messing with the flow rate but nothing helped)

If anyone could help that would be great, I need to print things for a fundraiser so I need it running. Thank you!

Do you have any pictures of what you call mushrooming? Your nozzle may be partially clogged, the insulator could be deformed, the feeder could be filled with grindded filament.
