Under-extrusion with internal shapes

Other than the problem described by an user here, I have another problem. On the outer part of the shell there is an underextrusion with the infill shape. Not only this, if there is an internal hidden hole, I will have an underextrusion with that shape too. The internal walls are fine, only the outer one is underextruded.
